To be registered you must live in the practice area. You will find a map on the "Contact Us" page.

 

You will need an appointment with our practice nurse. This will take approximately ten minutes.

 

You will need to bring to the appointment your medical card or a GMS! form, a completed health questionnaire and a specimen of urine.

If the appointment is booked by telephone we will require you to come to
the surgery to collect a registration form (if you do not have your medical
card), a health questionnaire and a specimen bottle prior to your
appointment. We will keep your medical card and a new card will be sent
to you in due course.
