| As a patient of this practice, you can expect: |
In exchange, we expect that you will: |
- to receive high quality, patient centred care
|
- attend appointments on time and inform the practice if you are unable to attend or wish to cancel or rearrange appointment
|
- to be involved in decisions about your healthcare and tobe given information to help you to do this
|
- try to contact the practice during the following periods of time:
everyday from 11:30 to 12:00, on Tuesdays, Thursdays and Fridays also from 7:30 a.m. to 8:00
if you have a routine query
|
- to be treated with dignity and respect by all practice staff
|
- understand that it will not always be possible to see the GP of your choice, particularlywhen booking an urgent appointment
|
- to get an urgent appointment at the practice within 24 hours
|
- allow up to 2 working days for the practice to process repeat prescription request
|
- to book a routine appointment or telephone consultation within 2 weeks
|
- treat all staff with respect and be polite and courteous when communicating with practice in person or by telephone
|
- to be referred to the hospital of your choice when clinically necessary
|
- only request a home visit if you are genuinely housebound through ilness, not if you lack transport to attend
|
- proactive treatment and management of oong term conditions and support to stay healthy
|
|
- that we will listen and respond appropriately to any suggestions, comments or complaints that you may have about the practice
|
|
|
